Sociants is a platform that connects social needs with community support, helping organizations identify social determinants of health and coordinate assistance in cases of extreme weather events or disasters. It gives organizations a way to actually see and act on the social factors affecting people’s wellbeing, not just their medical needs.
Based in Puerto Rico, the platform includes a digital directory of over 1,200 community providers, nonprofits, and agencies to facilitate referrals and collaboration, along with tools for creating and managing digital social assistance case records in a secure, privacy-compliant way. Its focus is on improving health and wellbeing at the community level by making social care coordination more effective.
